St. Anthony will be accepting online registrations. The link to the registration process will be located below. We will not be accepting paper registrations. If you have questions regarding this process, you may contact the coordinator for your sport.

Also, a CYO physical form must be completed before your child will be allowed to participate. A current, yearly physical form must be turned in prior to the first practice.  Copies of the CYO physical forms can be downloaded below. If you have any questions regarding these forms or their completion, please contact Jerry Towalski or the coordinator for the selected sport.

 

Concussions can be a very dangerous situation. Your coaches and coordinators believe that safety is a priority. Below you will find a downloadable .pdf on how to recognize concussions and steps to ensure a proper recovery.
